Salary: $100,000 - $116,800 per year

RN Clinical Coordinator / Hospice RN – Inpatient Setting
We are seeking experienced Registered Nurses (RNs) to join a hospital‑based hospice program as RN Clinical Coordinators / Hospice RN Case Managers supporting inpatient hospice units across multiple hospital campuses in Manhattan.
This opportunity is ideal for Hospice RNs, RN Hospice professionals, Registered Nurse Hospice candidates, or ICU / Med‑Surg / Oncology RNs looking to transition into a structured inpatient hospice role with predictable scheduling and strong interdisciplinary support.
Key Responsibilities
Serve as the RN Clinical Coordinator within inpatient hospice units
Coordinate care for patients at the General Inpatient (GIP) level
Collaborate with hospital physicians, case managers, and social workers
Respond to hospice referrals and gather clinical data for eligibility determinations
Coordinate symptom management prior to discharge to home hospice
Participate in Interdisciplinary Team (IDT) meetings and daily huddles
Develop and update hospice nursing plans of care
Educate hospital staff on hospice philosophy and care protocols
Oversee clinical care provided by hospital staff to hospice patients
Maintain accurate, compliant clinical documentation
Schedule & Location
4x10 schedule (8am–6pm)
Alternating weekends required
Float between three hospital campuses in Manhattan
Qualifications
Active RN license
Minimum 3 years of clinical nursing experience
Background in Hospice, Palliative Care, ICU, Med‑Surg, or Oncology preferred
Strong clinical judgment and communication skills
Comfortable working in a hospital‑based, interdisciplinary environment
